Carer Support Forms are accepted online – Send your claims to csclaims@health.govt.nz Requirements: only 1 email per claim, and the client number must be in the title of the email. You can find the client number on the top right of the claim form. Scan the claim first, then supporting documents such as receipts; the claim must be saved as a PDF. Check your document before you send it – if it’s not clear and you can’t read it yourself, the claim team won’t be able to either. Emailing your form doesn’t mean the claim will be processed any faster, but it will allow you to maintain your bubble without going to a post box. If you have questions call the Contact Centre, 0800 855 066. It is experiencing high call volumes so as with most things at the moment, please be patient.
